q Schedule your physical examination with your doctor.

q Make a copy of your Health Care Directive or Advanced Directive (Living Will) Form.

q Arrange for someone to drive you home.

q Communicate your special needs.

q Avoid smoking, street drugs or alcohol the day before your surgery.

q Complete any preparations as instructed by your doctor.

q Do not eat or drink the day of your surgery.

q Take your physician-approved medications, pain pills.

q Remove all jewelry, including for body piercing.

q Bring the following items:

q container for glasses, dentures or hearing aids

q copy of Health Care Directive (if staying in the hospital)

q toiletries

q reports from doctor

q insurance card and Social Security number

q Leave valuables at home.
